A miscarriage is the loss of a pregnancy before 24 weeks. While vaginal bleeding is one of the most common warning signs, bleeding or abdominal pain during pregnancy does not always mean a miscarriage is happening. Some women experience bleeding in early pregnancy and go on to have a healthy pregnancy.
Because symptoms can have different causes, anyone who is pregnant and experiences unusual bleeding or significant pain should seek medical advice rather than trying to determine the cause at home.
One of the most common signs associated with miscarriage is vaginal bleeding. This can range from light spotting to heavier bleeding that may soak a sanitary pad. The blood can appear pink, red or brown, and it may occur continuously or come and go.
Another possible warning sign is cramping or pain in the lower abdomen. Some women describe it as similar to period pain, while others may experience stronger or persistent cramps. Lower back pain can also occur alongside other symptoms.
Some women may also notice clots, lumps or tissue-like material passing from the vagina. Pink, grey or white material can sometimes be seen during a miscarriage. A sudden release of fluid from the vagina can also occur.
Another possible sign is a sudden change or loss of pregnancy symptoms, such as breast tenderness or morning sickness. However, this alone cannot confirm a miscarriage because pregnancy symptoms naturally fluctuate, and some women have few symptoms even during a healthy pregnancy.
Importantly, some miscarriages cause no obvious symptoms at all. A person may only discover that a pregnancy has stopped developing during a routine ultrasound examination. This is sometimes called a missed miscarriage.

There are also situations where symptoms that appear to indicate miscarriage could actually be caused by something else. Vaginal bleeding and abdominal pain in early pregnancy can occur for several reasons, including changes to the cervix, infection, implantation-related bleeding, miscarriage or an ectopic pregnancy.
An ectopic pregnancy is particularly important because it can become life-threatening. Warning signs can include lower abdominal pain, particularly on one side, vaginal bleeding, shoulder-tip pain, dizziness or fainting. A ruptured ectopic pregnancy can cause sudden severe abdominal pain and requires emergency treatment.
Anyone experiencing heavy vaginal bleeding, severe abdominal pain, shoulder-tip pain, dizziness, fainting or feeling extremely unwell during pregnancy should seek emergency medical care immediately. These symptoms can indicate a serious complication requiring urgent treatment.
For less severe bleeding or mild cramping, it is still important to contact a healthcare professional or maternity service for assessment. Doctors may use an ultrasound scan, pregnancy testing and blood tests to determine what is happening.
It is also important for women experiencing these symptoms not to blame themselves. Early miscarriages are often related to problems with the development of the pregnancy, including chromosomal abnormalities, and are usually not caused by ordinary activities such as exercise, stress or having sex.
Most importantly, symptoms alone cannot confirm that a miscarriage is happening. A medical assessment is needed to determine whether the pregnancy is continuing normally, whether a miscarriage has occurred or whether another condition needs treatment.
If you are currently pregnant and experiencing bleeding or abdominal pain, especially if the symptoms are worsening, contact a healthcare professional or go to the nearest hospital for assessment.
